What it's about
Henry Marsh, a British neurosurgeon, recounts decades of operating on brains, describing triumphs and catastrophic failures with unusual candor. The book argues that surgery is not a heroic science of certainty but a practice of probability, error, and irreversible consequence, and that surgeons must live with the guilt of their mistakes while continuing to operate. It mattered because it punctured the myth of the infallible doctor and gave the public an unusually honest account of medical fallibility.

What’s inside
- 1The Brain as Unknown Territory1 min
Brain surgery is fundamentally different from other surgery because even minor errors can destroy the very thing that makes a person who they are.
- 2Learning the Craft Through Watching Others Fail1 min
Surgical competence is built less on individual genius than on absorbing the visible failures of mentors and predecessors.
- 3The Ritual of Informed Consent1 min
The moment of obtaining a patient's consent is less about transferring information than about managing the unbearable asymmetry of knowledge and fear between doctor and patient.
- 4Living With the Ones Who Went Wrong1 min
A surgeon's career is defined not by successes, which fade from memory, but by the specific patients they harmed, who remain permanently present.
- 5Neurosurgery in Ukraine: Medicine Without Resources1 min
The quality of medical outcomes depends as much on institutional infrastructure and political conditions as on individual surgical skill.
- 6Bureaucracy and the Erosion of Medical Judgment1 min
Administrative and managerial reforms intended to make hospitals safer often degrade the working conditions that actually protect patients from error.
