What it's about
Sam Quinones traces how two parallel developments—the aggressive marketing of prescription opioids like OxyContin and the rise of a decentralized Mexican heroin distribution system from Xalisco, Nayarit—converged to produce mass addiction across white, middle-class, small-town and suburban America. The book argues the crisis was not caused by moral failure or urban poverty but by deliberate corporate deception, medical overconfidence, and an efficient, low-violence drug-delivery business model that exploited America's pain-treatment culture.

What’s inside
- 1The Pain Revolution1 min
American medicine's shift toward treating pain as a 'fifth vital sign' created the cultural and institutional opening that made mass opioid prescribing possible.
- 2Purdue and the OxyContin Machine1 min
Purdue Pharma's marketing of OxyContin as a safer, extended-release opioid was a calculated corporate strategy that manufactured demand ahead of evidence.
- 3Xalisco and the Black Tar Innovation1 min
A small Mexican farming county invented a heroin retail model that behaved like a pizza delivery service, deliberately avoiding violence and targeting new markets rather than existing drug scenes.
- 4Small Towns as Perfect Markets1 min
The epidemic concentrated in white, middle-class, small-town and suburban America because these places had latent demand, low suspicion, and social norms that masked addiction.
- 5The Doctors and the Pill Mills1 min
A subset of physicians became de facto drug distributors, and the medical system was slow to police them because addiction was misclassified as a legitimate pain-treatment outcome.
- 6From Pills to Heroin1 min
Crackdowns on prescription opioids around 2010 did not end the epidemic; they redirected already-addicted populations toward heroin, showing that supply-side enforcement without treatment simply shifts the crisis.
- 7Recovery, Stigma, and the Limits of Enforcement1 min
The epidemic exposed that treatment infrastructure and cultural attitudes toward addiction lagged far behind both the drug supply and the scale of dependence, leaving communities without real tools for recovery.
About Sam Quinones
Sam Quinones is an American journalist who spent a decade reporting from Mexico before covering the U.S. opioid crisis for the Los Angeles Times. He combined years of on-the-ground reporting in both countries—interviewing addicts, dealers, doctors, and DEA agents—to build Dreamland's dual narrative.
