What it's about
Adam Kay reconstructs six years of diary entries from his training as a British junior doctor, mostly in obstetrics and gynaecology, to show what the job actually costs. The book argues that the humor doctors use to survive brutal hours, low pay, and constant exposure to death is not callousness but necessity, and that an underfunded, understaffed NHS training system quietly breaks the people who run it. It mattered because it landed during the 2016 junior doctors' contract dispute and reframed a labor argument as a human one.

Who it's for
- Anyone who has ever waited too long in an NHS or hospital corridor and wondered who was actually running the place - People considering medicine, nursing, or any high-stakes shift-work career who want an honest account of the toll before they commit - Readers who like memoir that mixes slapstick with grief and don't need a tidy moral at the end
What’s inside
- 1Diary as Defense1 min
The gallows humor running through the book is a functional survival tool for people who face death routinely, not evidence of doctors becoming numb or careless.
- 2The Unpaid Hours1 min
Junior doctor training in the NHS extracts systematically underpaid overtime labor and calls it professional dues, which is a structural exploitation problem rather than an individual complaint.
- 3Understaffing Disguised as Error1 min
Many near-misses and mistakes in the book are caused by systemic understaffing, not by the incompetence of the individual doctor involved.
- 4Extreme Highs, Extreme Lows1 min
Obstetrics is uniquely brutal among specialties because it forces the same doctor to hold joy and catastrophe in the same shift, often in adjacent rooms.
- 5A Life Squeezed Out1 min
The training system's demands on time and location make ordinary adult relationships and family life structurally difficult to sustain, and this is a foreseeable cost of the system's design, not an individual work-life balance failure.
- 6Moved Every Few Months1 min
The rotational training structure, which shuffles junior doctors between hospitals and specialties every few months, destabilizes both clinical competence and personal life in ways senior doctors and administrators have normalized.
- 7The Case That Ends It1 min
The book's central claim about burnout is best understood through one specific catastrophic case, where systemic pressure, exhaustion, and lack of support combine to produce a preventable tragedy that the system has no real mechanism to help the doctor recover from.
- 8Testimony as Advocacy1 min
Turning private diary entries into a public book was itself a political act, intended to give the 2016 junior doctors' contract dispute a human face rather than a policy abstraction.
About Adam Kay
Adam Kay trained as a doctor in the UK, working his way from foundation year to registrar in obstetrics and gynaecology before quitting medicine after a traumatic case. He moved into comedy writing and performing, later adapting this memoir into a BBC drama. His diaries, kept throughout training, became the raw material for the book.
